Thaddeus Young scored 26 and Darren Collison added 25 as the Pacers scored their fifth victory against three losses.
Victor Oladipo delivered 23 points, while Bojan Bogdanovic added 17 for Indiana.
Domantas Sabonis had a double-double of 15 points and 12 rebounds.
The Cavaliers have lost to the Brooklyn Nets, New Orleans Pelicans, New York Knicks and now the Pacers.
LeBron James had 33 points on 14 of 22 shooting, and made 11 assists.
The Cavaliers also lost Tristan Thompson early as he hobbled off with a left calf strain in the second quarter.
Derrick Rose went 9 for 13 and wound up with 19 points, while Jeff Green came off the bench to deliver 15 points.
Kevin Love shot 4 for 12, including 2 of 6 from the free throw line, and finished with 13 points.
